We’re excited to have you as part of the A2F community! Before using our facility, please take a moment to review and complete the following waiver. This form is required for all members and guests to ensure everyone’s safety and understanding of our policies—especially if you plan to train alone when no staff is present.

Your safety, accountability, and personal responsibility are important to us. Please read each section carefully and sign where indicated.

Acknowledgment

1. Acknowledgment of Risk

I acknowledge that participation in physical exercise, fitness activities, training programs, and the use of A2F Gym equipment involves inherent risks. These include, but are not limited to: muscle strains, sprains, injury due to equipment misuse or malfunction, and in rare cases, serious injury or death. I fully accept all such risks, whether identified or not.

2. Assumption of Responsibility

I understand and accept that I am solely responsible for my physical condition, any limitations I may have, and my use of A2F Gym facilities. I agree to exercise within my own physical limits and to use equipment only as intended.

 

3. Solo Training Acknowledgment

I understand that A2F Gym may allow members to access the facility and train alone, without staff or other members present. I acknowledge the increased risks associated with unsupervised workouts, including the absence of immediate assistance in the event of injury, equipment failure, or a medical emergency. I assume all responsibility for my safety and conduct during such times and agree to take reasonable precautions, including but not limited to:

  • Having a phone available for emergency use

  • Avoiding max-effort lifts without spotters

4. Release of Liability

I, for myself, my heirs, assigns, and legal representatives, hereby release and discharge A2F Gym, located at 9542 Foothill Blvd, Rancho Cucamonga, CA 91730, including its owners, trainers, employees, contractors, and affiliated entities, from any and all liability for injury, illness, death, loss, or damage to person or property, including those caused by negligence, arising from my participation in any activity at the facility.

5. Medical Clearance & Consent

I affirm that I am in good physical condition and do not suffer from any known disability or condition which would prevent or limit my participation in physical activities. I understand it is my responsibility to consult a physician before beginning any fitness program.

6. Photo & Video Release 

I consent to the use of photos or video taken of me in the gym for promotional or educational purposes, unless I opt out in writing.

7. Agreement to Terms

By signing below, I confirm that I have read, understood, and voluntarily agree to the terms above. This waiver will remain in effect as long as I am a member or visitor at A2F Gym.
